  • Student Pick-up Authorization Form
  • Van Service Application Form, if any
  • Copy of student’s birth certificate
  • Copy of student’s passport if any
  • Copy of parents’ ID cards/passports
  • Copy of family book/proof of residence
  • Copy of student’s vaccination record
  • Copy of ID cards/passports of authorized pick-up persons
  • 5 photos of student, parents, and authorized pick-up persons
  • Payment of Application Fee of 50$ (non-refundable, if not enrolled)
